Welcome to our practice.
Our Physician and Staff welcome you to our offices. Our highest priority is to provide you with the
best medical care.
Our Physician is Board Certified in Internal Medicine and Cardiology, with practices limited to
Cardiology. The members of our Clinical Staff are trained to assist your Physician in providing
your medical care. It is important to us to make your clinic visit as pleasant as possible.
We are available to assist you and take your calls during our Office Hours 8:00a.m.-5:00p.m. Monday
thru Friday. Our office closes for one hour lunch during 12:00 noon to 1:00p.m. Our Clinic Hours
are from 4:00p.m. – 9:00p.m. Monday thru Thursday. Please call our McHenry office for office hours.
During after hours, weekends and holidays there is always a Physician on call when our office is
closed. If you need to make a call after hours, our answering service will help you reach the
Physician on call. Please instruct the answering service that you wish to speak with the Physician
on duty and leave your number. The Physician will respond as quickly as possible. Be prepared to
accurately describe your problem and list the medicines you are taking.
In the event of an emergency, CALL 911 to dispatch an ambulance. It is suggested that you advise
the ambulance driver to take you to the nearest emergency room. Once you are admitted to the hospital,
the emergency room staff will notify the cardiologist on call for Northwest Cardiology Associates.
Appointments can be made in person or by phone. We recommend calling six weeks in advance for
non-urgent visits. Patients requiring urgent visits will be seen within 48 hours of contacting
our office, this excludes weekends and holidays. Our Patient Coordinator in conjunction with our
Physician will help determine if your problem is urgent when you contact our office. We ask that
you arrive 15 minutes prior to your appointment so that our front desk and medical assistants can
prepare your visit with the Physician. We ask that you bring to your appointment: a picture ID,
insurance card(s), a list of your current prescription medications, over the counter medications
or vitamins, and patient forms. As a reminder, our Patient Coordinator will call you 24-48 hours
prior to your appointment. At this time, the Patient Coordinator may also ask for a list of
medications so that we can update your medical chart. If you are not able to keep your appointment
please give us 24 hour notice.
Despite our best efforts to remain on schedule emergencies may arise or patient’s care may require
more time than anticipated, or due to occasional interruptions by the hospital our schedule may be
delayed. We do apologize for any inconvenience that this may cause you. We will make every effort
to notify you of any changes and to reschedule your appointment as soon as possible.
We offer patient Prescription refills at every office visit. It is best if you bring a list of your
current medications to ensure timely refills. Should you require a refill outside of a visit to the
office please call our office during normal business hours. Please give us at least 72 hours advance
notice for the request. To obtain a prescription refill simply call 847.382.1608 between the hours
of 8:30a.m.-5:00p.m. You will speak with our Patient Coordinator who will need the following information:
- Your Name
- Date of Birth
- Address
- Phone Number
- Medication Name
- Dose
- Frequency
- Quantity
Patients must have been seen by Northwest Cardiology Associates within the past year for us to refill
your prescription. If you require an appointment to qualify for a refill, our staff will assist you in
scheduling an appointment. We only refill medications that we actively manage.
You may also call the pharmacy and request them to call our office for authorization on the refill. This
will facilitate your request in a timely manner. The Patient Coordinator will submit your request to
your Physician. Once the refill had been authorized, it will be called to your pharmacy. If there is
a problem with the request, you will be notified by our Patient Coordinator.
Our Patient Coordinator will facilitate in getting your Test Results ordered by your cardiologist for review.
Please allow two business days for test results to be available for review by your cardiologist. Your
cardiologist must first review your test before you are notified of the results. The Patient Coordinator will call
with your test results along with the cardiologist’s recommendations. If you have a test just prior to
a scheduled office appointment with your cardiologist, the test results will be reviewed in detail at
the time of the appointment and you will not receive a phone call.
